This portal supports care requests, shared appointments, and administrative reminders for About Your Care Home.

Information you share

Your account includes your name, email address, and any phone number you provide. Care requests include the care recipient’s name, address, and your message. Please share only the information needed for an initial conversation; do not include medical records or identification documents in a request.

Who can see it

Your request is available to authorized administrators. Information about an established care relationship is shared with authorized family accounts and assigned team members according to their permissions. Internal administrative notes are separate from the information shown to families.

Appointments and reminders

Reminder recipients and preferences are configured separately. Email and text reminders should contain a brief notice and a link to sign in, rather than detailed care information.

Account information

You can update your contact details in your account. Speak with the care-home team about corrections, access, or information that should no longer be shared.

This notice will be completed with the business’s confirmed contact details and retention practices before accepting live care information.
